Terms and Conditions for MyChart

Deaconess Health System and its affiliates who use the EPIC Medical Record System are pleased to bring you MyChart — your electronic health record. By opening a MyChart account, you agree to be bound by the following Terms and Conditions. Deaconess may change these Terms and Conditions at any time and will post new Terms and Conditions on the MyChart website. New Terms and Conditions are effective as soon as they are posted.

Communications with your physician: If you send a MyChart message to your physician, it will first be read by nurses working in your physician's office. The nurses may respond to you directly or may forward your message to the physician for further review and response. You should allow two business days for a message response.

Urgent communications: MyChart should not be used for urgent communications with your physician. If you have an urgent health concern, you should contact your physician office by telephone. If the issue is an emergency, call 911 or go to an emergency room.

System security: The security of your information is important to us. My Chart is accessed through a Secure Sockets Layer (SSL) compatible browser and the application utilizes 128 bit encryption to protect your information. You can tell when you are in secured areas of the site by looking at the URL in your browser window. If the URL begins with https:// then you viewing secured information.

In addition, you access MyChart using a unique user name and password. Your user ID and password are stored in an encrypted database. It is your responsibility to protect your password. If you share your password with others, they will have the ability to access your records.

E-mail privacy: Communications in MyChart are protected as described above. When you receive a MyChart message from your physician, you will be notified through your personal e-mail account that you have a message waiting for you on MyChart. The e-mail will not contain any confidential medical information. Anyone having access to your e-mail account will be able to see that you have a MyChart account and could reasonably guess that you are a patient of Deaconess or a Deaconess affiliate.

Access to MyChart:

  • Adults: Access to MyChart is granted to persons age 18 or older who are patients at a Deaconess physician's office or the office of a participating non-Deaconess physician who uses the EPIC medical record system.
  • Parents of minor children: Parents (biologic or adoptive) of children under the age of 14 may request access to their children's records. Because children aged 14 and up have the statutory right to request certain medical services independent of their parents, parental access will be limited to the ability to make appointment requests and send or receive messages once the child turns age 14.
  • Minors: We do not at present offer persons under the age of 18 access to their own records.
  • Proxy access: Patients may elect to grant access rights to other persons called proxies. For example a husband may request that his wife be granted proxy access to view his records and to make communications to his doctor on behalf of her husband.
  • Persons who are court-appointed guardians, power-of-attorney, or designated health care representatives for incapacitated patients may request proxy access to a patient record. In all such cases, the requesting party must submit documentary proof of their appointment as guardian, power-of-attorney or health care representative.

Discontinuation of Access: Deaconess reserves to itself the right to terminate a person's MyChart access at any time and for any reason. Some common reasons for termination include but are not limited to:

  • Death of the user
  • Prolonged period of inactivity of the MyChart account
  • Abuse of the account - For example, use of vulgar or threatening language in communications to the physician, repeated attempts to communicate with a physician who has severed the physician/patient relationship.

Patients with a MyChart account who have granted proxy rights to others have the ability within their own account to terminate the proxy access. In the event that a patient does not terminate a proxy's access, Deaconess assumes no responsibility for the proxy's continued access to the account. Patients who are unable to terminate a proxy's access must contact the MyChart HelpDesk at 812-450-EPIC M-F 8AM - 5PM to request proxy access termination.

Medical record: The MyChart record is not a complete copy of your patient medical record. For example, pathology results will not be provided through MyChart. MyChart users and persons who do not have access to MyChart can obtain copies of their medical record by contacting their provider or the Medical Records Department of Deaconess Hospital.

Parents of children over the age of 14 may request to view or obtain copies of their child's records from the treating physician's office or the Medical Records Department of Deaconess Hospital.

Site availability: MyChart may become unavailable during periods of site maintenance and backup or in the event of a site crash. Access is provided on an ‘as available’ basis and Deaconess does not guarantee access at any time.
